Booking your vaccination

Vaccination is available by booking an appointment or by walk-in (no booking required) at select vaccination clinics.

To make a vaccination booking please use the online portal or call 1800 675 398.

Before you make your booking

Read the COVID-19 vaccination consent form

• If you answer ‘yes’ to any of the consent checklist questions, you may need to consult with a health provider prior to vaccination.
• The consent form is available in multiple languages.

Read the information sheet for the Pfizer, Moderna, AstraZeneca or Novavax vaccine to ensure they are appropriate for you
